The City Clerk is the City’s Records Official. Therefore, all requests for Fire Department information, records, research, etc., will need to be submitted to the City of North Las Vegsa City Clerk’s Office, including the following record types:
- EMS (Medical Aid Reports)
- Fire Incident Reports
Requesters are notified of the appropriate amount due (if applicable) once a request has been completed. Duplicate submissions do not expedite a request. Documents are released to the requester once payment has been received. Incomplete forms will be returned to the sender, resulting in a delay with the request. Click here for the Online Request Form.
The City requires a notarized release signed by the patient for EMS (Medical Aid) reports prior to releasing any records and/or information to anyone other than the patient. If a notarized release is not obtainable, a court-issued subpoena is required. Forms used with requests for Fire Department records are published on the City Clerk’s online forms page and linked to the online request form.
